having break for a while before have the common session at 2.30 pm later..

ok,currently im in orthopedics posting, week 4 dah pun..and tomorrow will be our last day in Hospital Serdang where after this we will continue our posting in Terendak army hosp for 3 weeks..

what can i say about ortho posting?
scaryyy...
yuhuu...for me it's scary.
for my friends, they all seems to be happy slalu..yeah,coz ortho la paling lepak-skali-posting-ever.the orthopedics specialist sume serupe je perangai,cool and steady..so,cool la kan?hehe

why im scary?
sy takot nak gi ward ortho,sbb kebanyakan patient ortho sgt menakutkan sy..hua!!!!
wpon da week4, i still have the feeling..
let me list the common2 patient in ortho ward..


1. DFU (diabetic foot ulser)
:slalu org ckp hospital adalah tmpat untuk potong2..yeah,u r rite anyway..masuk ada kaki,kluar hosp xde kaki..kbykn ptn kencing manis mcm tu la..know what, i can't stand the smell of ulcer+pus+blood+gangrene+dead tissue+worms yg mmg byk sgt dlm ward tuh!!!!!hukz..if only u can see those type of foot, u will agree with me..ive to use the mask (while my friend relaks je berdiri sebelah kaki pak cik tu), sbb x tahan sgt dgn bau dia..memang ptn kencing manis xkan perasaan any cut or luka kat kaki mereka (neuropathy), but then after a while, they will develop all this pathological condition make them worsening smpaikan if they didn't chop the leg,nyawa boleh melayang as easy as that. sanggup nk tgk DFU?link sini.. ni baru sikit,nk tengok yg teruk2 jom pi ward..


2. trauma ptn (accident)
:ni lagi scary dr DFU..yesterday when i was on-call with my friends in the A&E department, there was 3 cases of orthopaedics. the most scariest case was a 5 year old boy was digilis oleh van yg x perasan the presence of the boy at the back of the van. the right lateral thigh was beaten up by the tyres, and fortunately mak budak tu nmpk and jerit suh driver tu stop the van.OMG!!!!!
5 year old boy,man...bawah van..daging kaki kanan da loose, even the bone doesn't fracture. kat tayar tu bersepah2 daging kaki budak tu( as the dad told us)...
so,initially the hero (MA of course) will clean up the wound & set IV.the A&E dpartment bergegar coz budak tu jerit sekuat tenaga..hukz.of course they already bg pain killer, antibiotic and stuff..but still,kite yg nengok ni pon nak nanges rsnye..HO and the MO took history from the parents and took the pic of the leg.we all the med student berdiri tegak je kat tepi dinding, good observer la kan..muke the doctors was like cool je,x nmpak riak cuak..slamber je take history and stuff..but i bet,dlm hati tu,ya Allah,cmne aku nk tahan perasaan ni..
tu la,doc have to be a good lier.not the penipu mkn duit org tu,tp berlakon to be good.kalo doc tunjuk mke sedih,kasihan,rs nak nagis,how come the parents nak bg kepercayaan,is it?

ini baru satu kes.emergency.kat ward tu,lg byk kes trauma,but the ptn da stabilize la...today ill stay in A&E lagi after maghrib..to build my strength..huhu..kasik semangat naik k mimi!


basically 2 types of ptn ni shj yg akan menjadi 'permainan' ku dlm masa 4 weeks time..

p/s:i feel like i want to be a superhero u know..i want to prevent any cut, any accident, any trauma to all patient, so that, they will nor presented to the hospital with those condition. can i?

1. panaskan minyak, and goreng daun panda ( cm x penah buat)
2. masukkan jintan manis, bunga cengkih, bunga lawang
3. pas da wangi,masukkan halia, cili padi, bwg merah and bawang putih yg da dihiris2
4. masukkan ayam
5. ketuk serai..masukkan dlm kualai, masukkan skali daun kari and tomatoes
6. pas da kire2 masak tu, masukkan rempahserbuk, serbuk kunyit and serbuk cili
7. lastly, garam...

balong baru balik dari MIAT (tp nmpknye dia kene balik skool balik,prepare nk final), nana baru balik dr hostel,ain and hakim yang ada kat umah slalu, with my both parents..and i felt happy with them around me..skrg hakim sedang mendendangkan lagu by Barney and friends (i ajar dia nyanyi lagu ni tau=)

pagi2 tadi,kul 6.45 am akim called me dengan excitednye nk borak2..tau la skrg ni dia x gi umah cumai (nursery) lagi, happy bukan main,bangun pun awal..kalo kene gi umah cumai,kul 11am pun lom tentu bangun lagi..bijak2..this little handsome boy memang smart..

p/s : best kan ada adek laki cmni..hehe..i wish he's not growing, i mean,dia stay comel and kiut and perangai dia mcm tu je smpai bila2..xnak dia besar nnt,ikut perangai dia,xleh nak manja2 cmni...sob2...

tepuk2 (i like!!)..hehe

well,today am still in KK Dengkil,and tag with Dr Hafiza.best sgt,syukur..she ask simple2 ques je..der,kiteorg ni boleh terlupa!!year 3 nye subject tu..haha..kantoi2 slalu main..isk2

Dr Hafiza ni MO (medical officer) MCHC (maternal child) kat KK Dengkil.lembut ok org dia..married with 2 children, wife to an engineer, product from UM.. byk gak kami berkongsi cerita, ala kat KK xramai patient ptg td, so mang best la duk ngn Dr nih..

jadiknye, patient yg dtg pon adalah golongan2 ibu2 mengandung untuk buat mecial check-up or ibu bw anak nak injection.im very proud to see husband2 pon duk sama beratur amik giliran,teman wife masing2, pgg baby, and those things..hehe..meaning,still man in Msia is very good in term of relationship with family..sanggup amik MC just to temankan anak dia minum ubat cacing..sanggup amik short leave just to hold the baby while the mother doing their consultation..isn't it a great view in KK!

owh,bahagia nye rasa hati..hehehe
well,ada jugak preggy mother yg terpakse naik bas sendiri,jalan kaki naik bukit (KK Dengkil atas bukit plak tuh) semata2 nak wat medical check-up nih..sian..husband keje jauh..majo yg dtg sendiri ni pun is from a poor family background.low socio-economic..sedey2..rasa mcm nak tlg amik and anta kan je patient yg x mampu nih..

and don't be surprise to see patient yg abused by his husband..mintak dijauhkan kan?hem..cmne husband tergamak sakitkan isteri dia ek?that's why i keep berdoa not to have a hot-temper man..xnak yg ske mara2..hukz..takotzz..

agak2,kalo preggy dr yg nak wat antenatal check-up ni,ada x husband nak ikut teman?(hem,wondering gak nih)..huhu

k la..nak list down pe yg dr tu tanye ari nih..(mls nak tulis dlom log buk)

1. what is the average weight gain by the pregnant mother?
for the first 5 month,should increase 1 kilo per month. and 0.5kg for the consecutive week.

2. what is physiological jaundice?
immature liver.(senang ingat,sila explain sendiri)

3. what is GDM and indication for MOGTT?
gestational diabetic mellitus (GDM) : mom dpt GM in pregnant, b4 pregnant xde pon DM..
so,screening using 75gm glucose +250ml water at least once at >24 weeks..OGTT values for pregnant lady same with impaired glucose intolerence (IGT). pre:6.1-6.9..post: 7.8 - 11.0
indication:

• BMI>27
• previous big baby or weigh 4kg and above
• previous GDM
• 1st degree relative with DM
• bad obstetric history
• glycosuria at 1st prenatal visit
• current obs prob( essential HT, PIH,polyhidromnions, current use of steroid)
• age > 25

4. what is PIH?
pregnancy induced hypertension where mom's bp is >140/90mmHg, at 20 completed week.(hapal minggu tu skali)

5. what is placenta previa?
the placenta attached to the uterine wall close to or cover the cervix, at 28 completed week.


6.what do u check in 3 month preggy mother?
3 bulan mommy baru buat booking date,buku merah and baru buat sume screening test, why 3 month?bcoz da confirm baby dlm perut, no abortion or miscarriage..ok,hem kite nak check for placenta previa, wpon PP can occur at 22 week (ikut WHO)..bleeding at this time of period,suspect PP..

k tu je la perkongsian ari ni..hehe,,
